Where can I get an application form?
Applications for the Porirua Youth Awards are accepted now until Friday 31 August 2018
Application Forms for the  Porirua Youth Awards are available from the Partners Porirua website or offices at L05 Lydney Place South, Porirua (next to ACC) or you can click here  (right click on the pop-up window, choose 'save as' or 'print').

 How to apply for a Porirua Youth Award

  1. Read through the application form and notes to get a good idea of the information needed.

  2. Arrange your ideas in a good order that is easy to follow.
    Answer the questions in a way that will help the judges to understand what you have already achieved and what you are trying to achieve with this award.

  3. Applications must be on this form.
    Please print neatly and clearly and attach additional pages as required.

  4. Make sure you complete the declaration and sign the application form.

  5. At least one written reference must be provided.

  6. Make sure you return your application to Partners Porirua, L05 Lydney Place South, Porirua 5022 or PO Box 50-462, Porirua 5024 by the stated date.
